Reject: Wrong Vaccine (vial or pre-filled syringe)
Reject: Wrong Vaccine Dose Prepared
Reject: Wrong Needle Length (vial)
Reject: Wrong Needle Length (prefilled syringe)
Reject: Precaution due to Illness, Allergy or RPh Note
Note: pay close attention to any patient allergy or chronic condition and check to ensure the vaccine can still be safely given.
Reject: Precaution due to Screening Question

Reject: Consent Form Not Signed
Reject: Vaccine Product Documentation Incomplete
