INSTRUCTIONS
- The goal of this activity is to become familiar with the iSBARR communication format by critically analyzing flawed examples.
- For each activity (renal, indication, IV-to-PO), carefully read the iSBARR communication.
- The drug names & doses are all fictitious. Do not focus on the accuracy of the therapeutics being discussed. Instead, focus on the effective communication of a recommendation between pharmacist and prescriber.
- Errors or omissions may occur in any or all of the following sections:
- (I)ntroduction
- (S)ituation
- General (B)ackground
- Specific (B)ackground
- (A)ssessment
- (R)ecommendation
- (R)epeat Back
- Each time the page is refreshed, it will randomly generate information and determine whether a section will contain an error. Therefore, it is possible for an activity to have no errors, errors in all sections, or more likely a mixture of both errors and correct statements.
- Text will turn different colors depending on your choice:
- Green = correctly identified the section as having NO errors or omissions
- Red = correctly identified the section with as having an error or omission
- Blue = did not correctly identify the section
- If you make a single mistake on any section (i.e., selection turns blue), you must refresh the page and repeat the activity!
- It is expected that you will not find all the errors the first time attempting an activity. Be sure and look at the answer key for each activity, either before attempting or after failing to correctly identify all errors.
- WARNING: looking at the answer key to any activity prevents you from submitting any further answers for that activity. So if not completed, you will need to refresh the page and start again.
- Good luck!
